Caretaker of Children

In case you or your children’s parent(s) do not survive until your child’s 18th birthday, please advise who the caretaker of your children should be. Please indicate at least two potential caretakers, one primary and one secondary in case the first is not able to take care of your children. Please list this person's full legal name, address, phone number, and relation to child.

Businesses

Please list the name of your businesses, the business's federal or state ID number, and any co-owners of the business.

Business Account

Please list the name of your business account's institution; the type of account it is (savings, checking, etc.), and the last four digits of the account number. List multiple, if necessary.

Movable Property

This is any property that is tangible and can be given away (think paintings, classic cars, collector items, heirlooms). This section is anything you specifically want to be given to a specific person. Give as much detail as possible. You may include a conditional event such as “In the event I die without my children…”. You may also include a suspensive condition such as “_____ must survive me for _____ years”.

Immovable Property

This is any property that is not tangible (think homes (list address), properties, retirement accounts, IRAs, pensions, saving, checking account). Include the name, establishment holding the account, and the last four digits of the account. Give as much detail as possible. You may include a conditional event such as “In the event I die without my children…”. You may also include a suspensive condition such as “_____ must survive me for _____ years”.

Executor/Executrix

This person is the designated individual who is responsible for managing the affairs of a deceased person’s probate estate. Please list this person's full legal name and DOB.

Substitute Executor/Executrix

This person is the secondary designated individual who is responsible for managing the affairs of a deceased person’s probate estate, should the former executor / executrix be unable to do so. Please list this person's full legal name and DOB.

Residuary Estate
